The objective of this project is to conduct a multi-center, cluster-randomized study utilizing a centralized Cardiovascular Risk Service (CVRS) managed by clinical pharmacists. Twenty primary care sites with large geographic, racial, and ethnic diversity will be randomized to either the CVRS arm or a usual care arm. Each site is expected to enroll 20-25 patients for a total of 400 subjects, of whom 240 will be racial minorities.
